This course is for patients who have Type 2 diabetes or pre-diabetes and want to learn how to manage their condition through lifestyle changes such as diet, exercise and stress management.
The course is delivered by a Health & Wellbeing Coach in a small, supportive group setting.
Type 2 Diabetes and Pre-Diabetes – what’s the difference?
Pre-diabetes means your blood sugar levels are higher than normal but not high enough to be diagnosed as Type 2 diabetes. Without changes to diet and lifestyle, pre-diabetes can develop into Type 2 diabetes.
Type 2 diabetes is a condition where the level of sugar in your blood becomes too high. It is usually lifelong, but it can often be managed well through lifestyle changes and medication if needed.
This course is suitable for people with either Type 2 diabetes or pre-diabetes.
What the Course involves
- Four 1-hour group sessions
- Information about diet, exercise and lifestyle
- Support to help you build healthy habits
- Advice on managing blood sugar levels
- Opportunity to share experiences and learn with others in a supportive environment
The course is for patients who:
- Have Type 2 diabetes or pre-diabetes
- Are registered with a Brighton & Hove GP practice
- Want support to make lifestyle changes to improve their health
