The NHS Health Check is a free check-up to help understand your overall health and reduce your risk of developing certain health conditions.
The check looks at your risk of conditions such as:
- Heart disease
- Diabetes
- Kidney disease
- Stroke
During the appointment, you will be given advice and support to help you reduce your risk and stay healthy.
Who is eligible?
You may be eligible for an NHS Health Check if:
- You are aged 40 to 74
- You are registered with a Brighton & Hove GP practice
- You have not already been diagnosed with certain conditions such as heart disease, diabetes, kidney disease, stroke or high blood pressure
If you are unsure whether you are eligible, the team will be able to check this for you.
If you are aged 65 or over, you will also be given information about the signs and symptoms of dementia and where to get further support if needed.
How to book
You can refer yourself for an NHS Health Check by completing the self-referral form.
Once your form has been received, a member of the team will contact you to arrange an appointment at a local clinic.
Information for Patients
Patients who are eligible for an NHS Health Check will be offered an appointment at a local clinic. The appointment will include checks such as blood pressure, cholesterol and lifestyle information.
After the check, you will be given information and advice about how to reduce your risk and improve your health. Information will be shared with your GP practice so your records remain up to date.
